Do you have a planned psychedelic experience? Is your goal to process, understand, or heal from trauma? If so, take the following three surveys and learn about your readiness and potential risk for taking a psychedelic, as well as changes in your self-perceptions and mental health across time.

Automated Scored Reports
Each survey generates an automated scored report, summarizing your results upon completion. These results are categorized into different sections, depending on what survey you take. If someone is monitoring and/or facilitating your psychedelic experience, such as a therapist or retreat leader, you have the option of sending them a copy of your scored report via email, by entering their unique code.
Survey Suitability
This is a set of three surveys, which focus on trauma. If you have post-traumatic stress disorder (PTSD) or have experienced any form of trauma (e.g., physical, sexual, or emotional trauma), whether directly or indirectly, which includes neglect, discrimination, military combat, and incarceration, then these surveys are suitable for you to take. Otherwise, consider taking the “General Surveys” or another set of surveys that are available on our homepage.
Survey Timelines
These surveys should be taken in chronological order. The “Baseline Pre-Survey” should be taken first, roughly one week before your psychedelic experience. This captures your baseline status, which is used as a reference point. The “1 Week Post-Survey” should be taken second, roughly one week after your psychedelic experience. This captures acute change in your baseline status. Finally, the “1 Month Post-Survey” should be taken third, roughly 1 month after your psychedelic experience. This captures long-term change in your baseline status. You are highly encouraged to take all three surveys.
Survey Outcomes
These surveys are designed to improve your self-knowledge and understanding, which may help you prepare for, enhance, and integrate your psychedelic experience. Specifically, they may help you answer questions like: “Am I mentally ready for this psychedelic experience? Have I adequately prepared my physical environment? Am I doing this alone, or do I have someone to support me before, during, and afterwards? Do I know what physical, visual, and emotional changes I may experience, how to best handle them, and how long they may last? Do I have any medical contraindications to taking this psychedelic? How will this experience impact my trauma? What will I learn or discover about myself?
Survey Questionnaires
These surveys include various scales, or questionnaires, that are validated by scientific researchers. Below, you will find a list of questionnaires and a description of what they measure, as well as when they are administered.

This is measured by the “Adverse Childhood Experiences Questionnaire” (ACE-Q), a 10-item questionnaire that assesses traumatic childhood events, such as physical, sexual, and emotional abuse, as well as family dysfunction and neglect.

This is measured by the “Life Events Checklist for DSM-5” (LEC-5), a 16-item questionnaire that assesses traumatic life events, such as natural disasters, serious accidents, physical or sexual assaults, life-threatening illness or injury, and combat exposure.

This is measured by the “PTSD Checklist for DSM-5” (PCL-5), a 20-item questionnaire that assesses symptoms of post-traumatic stress disorder (PTSD), including re-experiencing events, avoiding trauma, negative mood and thoughts, and changes in arousal and reactivity.

This is measured by the “Post-Traumatic Maladaptive Beliefs Scale” (PMBS), a 15-item questionnaire that assesses abnormal beliefs following exposure to trauma, including threat of harm, self-worth and judgment, and reliability and trustworthiness of others.

This is measured by the “Post-Traumatic Cognitions Inventory” (PTCI-9), a 9-item questionnaire that assesses thoughts and beliefs following exposure to trauma, including negative thoughts about oneself, negative thoughts about the world, and self-blame.

This is measured by the “Trauma Coping Self-Efficacy” (CSE-T) scale, a 9-item questionnaire that assesses self-efficacy, or one’s belief in their capacity to cope with trauma.

This is measured by the “Post-Traumatic Growth Inventory” (PTGI), a 10-item questionnaire that assesses growth following trauma, including personal strength, new possibilities, improved relationships, spiritual growth, and appreciation for life.

This is measured by the “Dissociative Experiences Scale Taxon” (DES-T), an 8-item questionnaire that assesses pathological dissociation, or the likelihood of having a dissociative disorder.
